Typically the game proceeds until a stoppage or obvious slow down in play, unless the injured player or other players are in severe danger.
Ex1: Team A player rolls ankle, while Team B fast breaks to other end and score. Finish possession, then allow player to sub.
Ex2: Team A player takes blow to the head, or has a cut and is bleeding. Stop game immediately for sub.
Basically, unless the player require immediate medical attention or becomes a danger to the other players in the game, you allow play to continue. It may sound bad, but it helps prevent instances of faking injury to prevent an advantage, ie: what's happening in football to slow down Oregon-style offenses. Also, sometimes a player may get hurt and just need a few moments to gather themselves, rather than needing a sub and is forced to miss game action.
